Potential of focused ultrasound in epilepsy surgery

چکیده
Background/introduction The minimally invasive nature of FUS makes it an excellent candidate technology for epilepsy surgery. The limitation of currently available FUS systems is that the lesion must be deep in the brain to avoid bone heating. Thus, it is best suited for “subcortical” epilepsies, i.e. epilepsy resulting from lesions below the cortex. It is also desirable that the lesion be relatively small volume to avoid long treatment times and not be in eloquent cortex. Previous reports of seizure freedom after laser ablation of hypothalamic hamartomas and radiofrequency ablation of single subcortical nodules have been reported. Thus, we designed the Focused Ultrasound Subcortical Epilepsy (FUSE) study with these principles in mind.

PET and MRI-guided focused ultrasound surgery for hypoxic-tissue ablation combined with radiotherapy in solid tumors
Background: The rationale was to develop an ablation approach to destroy regions of tumor resistant to radiation and thus reduce the time required for whole tumor ablation, while improving overall tumor control after radiotherapy. Materials and Methods: The system is composed of a micro positron emission tomography (mPET), 7T magnetic resonance imaging (MRI), and a customized MRI-compatible foc...

Background/introduction Temporal lobe epilepsy (TLE) is the most common pharmacologically refractory form of epilepsy. While it can often be effectively treated by temporal lobe surgery, that does not always eliminate seizures, and many patients are not suitable candidates for surgery. A noninvasive method to augment surgical or medical treatment of TLE would be highly useful. Low-intensity foc...

INTRODUCTION Magnetic resonance imaging (MRI) is increasingly used intraoperatively, because it provides excellent soft tissue distinction and on-the-fly adjustment to imaging slice orientation without exposing clinician or patient to radiation. For neurosurgical procedures, intraoperative MRI holds significant advantages over traditional stereotactic methods (e.g. compensation for brain shift)...
متن کاملI-49: Safely Extending Focused Ultrasound Surgery for Uterine Leiomyomas to Women Who Desire Future Pregnancies
Background: Focused ultrasound surgery (FUS) is a novel noninvasive surgery that results in localized tissue necrosis without any breech of the skin or other body orifice. MRI guidance provides anatomic visualization and real time thermal feedback. Our goal is to safely extend this therapy to women who desire future pregnancies. Materials and Methods: We have undertaken 4 multisite trials of FU...

Introduction: Light-driven cancer therapy strategies (e.g. photodynamic therapy and photothermal therapy) have undergone remarkable progress in recent years, but they still suffer from a serious drawback of limited penetration depth of light in tissue. As a non-invasive and non- ionizing radiation, ultrasound can be focused remotely, transferring acoustic energy deep in the bo...
